What is Healthy Horizons?
Healthy Horizons is an exercise referral programme for customers with recognised medical conditions including a BMI of 30+ and associated health problems. The scheme is designed to offer clients a specialist exercise programme to improve or manage medical conditions.  Each client is assessed individually and a specific programme of activity designed to suit the individual needs.

Once you have completed the programme the instructor will carry out a full health assessment again and help you to identify the progress and improvements you have made.  You will then be offered a number of options as to how you can best continue with your new activity lifestyle.

Speak to your GP or practice nurse and ask about a Healthy Horizons referral, if they are unsure contact your local New Forest Health and Leisure centre and a member of the team will be able to advise you on the next step. www.newforest.gov.uk/healthandleisure
